1095-B Tax Form
When to expect your 1095-B Tax form (Proof of minimum essential coverage under the Affordable Care Act)
For actively working employees and early retirees covered by UPREHS. Your form will be mailed by the end of February, 2017. Please note, if you also have dependents covered under the Railroad National Health and Welfare Plan you will receive two forms, one for yourself and one for your dependents. If you need a replacement form for yourself, please contact Railroad Enrollment Services at 800-753-2692.
For Medicare members, Medicare is sending a Form 1095-B to people who:
- Are under the age of 65 and have Medicare Part A (Hospital Insurance);
- Enrolled in Medicare Part A for the first time in 2016; or
- Had Medicare Part A for part of 2016.
If you need a replacement form 1095-B, please call 1-800-MEDICARE.
If you had Medicare Part A for all 12 months of 2016, you’ll just need to check a box on your Federal income tax return indicating you had qualifying health coverage.
Important information: Taxpayers do not need to wait to receive Forms 1095-B before filing their returns
