Special plans for retired employees including Medicare supplemental are designed to give members benefits similar to those they enjoyed when they were working. Members are requested to contact the UPREHS Customer Service at (800) 547-0421 or (801) 595-4300 at least two months before retirement for plan qualifications.
UPREHS offers:
NATIONAL EARLY RETIREMENT MAJOR MEDICAL BENEFIT PLAN (ERMMB)
ERMMB provides members 60 years of age with 30 or more years of qualified Railroad service and not yet entitled to Medicare with the following benefits:
- No charge for coverage
- $100 annual deductible and then claims are paid at 80% of the allowed fee (Behavioral Health is paid at 65% with certain restrictions)
- Benefits are similar to those of an active member
- Maximum lifetime amount payable by UPREHS is $101,200
- Enhanced prescription drug benefits through our own Medicare Part D Prescription Drug Plan
60/30 PLUS
This is an optional plan for members who are qualified for ERMMB. It provides:
- Payment of 100% of the contracted amount if the UPREHS network is used
- If non-network providers are used, UPREHS pays 40% of the plan allowable
- Prescription drug benefits are capped at $1,700 annually
- Lifetime maximum limit of $326,200
- $15.00 Co-pay office visit / $50.00 Co-pay E.R.
REDUCED AGE AND DISABILITY
This plan is designed for members under 65 years of age and not eligible for coverage under any other UPREHS plan.
- Lifetime maximum limit of $150,000
- Benefits are similar to those of an active member, except there is no coverage for alcohol-related and mental disorder services
- No out-of-network benefits
- Prescription drug benefits are capped at $1,700 annually
- $15.00 Co-pay office visit / $50.00 Co-pay E.R.
SEPARATED EMPLOYEE COVERAGE
This plan is for members who have taken a separation payment (buy out) from the Railroad and are within 10 years of retirement.
- Benefits are similar to those of an active member
MEDICARE SUPPLEMENTAL PLAN
This plan is for members who are enrolled in Medicare Parts A and B.
- UPREHS benefits supplement most allowed Medicare payments
- 100% of co-payment and deductible amounts are paid for network services
- Co-payment and deductible amounts are paid at 40% for out-of-network services
- Prescription drug benefits are capped at $3,000 annually
- UPREHS processes Medicare claims for network physicians
MEDICARE SPOUSE COVERAGE
This plan is available to a Medicare spouse of a qualified retiree. To qualify, the spouse must be enrolled in Medicare parts A and B and must enroll at the time he/she becomes elig. for Medicare..
- Benefits are the same as the Medicare Supplemental Plan.
